The role of Discharge Support Volunteer provides practical, emotional and social support to patients once back home. The aim of the role is to help reduce the fear and anxiety that vulnerable patients may experience upon leaving hospital. As a result, you will play an integral part in providing comfort and reassurance. The scheme is also aiming to reduce hospital readmissions.
Discharge Support volunteers provide a one off telephone call on the patient’s return home to build confidence and encouragement to access wider support in the community. Referral to other services can also be arranged if necessary.
What will I be doing? Based at Southport hospital you will:-
• Telephone patients 48 hours following discharge.
• Provide information, signpost and refer to community, voluntary and other support services if required and monitor these referrals
• Update paperwork and monitoring system
AM and PM shifts; no weekends.
KNOWLEDGE; TRAINING & EXPERIENCE REQUIRED TO DO THE ROLE:
Aged 17 or over
Good communication skills
Warm and friendly manner
Ability to work in a team
Ability to follow instruction
Ability to empathise and show compassion
To complete any specialist training which is relevant to role
Commitment to uphold Trust core values and NHS policies
